8/9/2023 EnchantaVaughn Approved Behavior: Anthony is afraid he will be judged for things he cannot control, and he attempts to hide his feelings from everyone in the home. Anthony expressed resentment for not being able to be open with his mom during his difficult moments and was sad about it. Anthony was supported for being honest. Anthony was given a 20-minute break to process his thoughts while mom and counselor discussed important information regarding his treatment plan. As the Anthony has been allowed to have greater choice regarding his services and providers, he has been more motivated in treatment. Behavior: Anthony continued to exhibit restless, hyperactive, fidgety and uncontrollable manners, as it was complicated and difficult for Anthony to remain in his seat throughout the second half of the session. Anthony was prompted to try his best to get through the session and to relax and pay attention. Anthony continued to show guarded behaviors. Anthony communicated with QMHP-C that he cannot open up and express himself around his family in the same way he can open up and express himself to his counselor. Intervention: QMHP-C held consultations with Anthony and mother about effective intervention strategies that build on the Anthony's strengths and compensate for his negative behaviors. QMHP-C used the basic listening sequence to clearly draw out the point of view that the Anthony was feeling and get beyond his aggressive language and behaviors. QMHP-C implemented individualized behavior management techniques with Anthony. QMHP-C helped Anthony develop healthy self-esteem by reducing level of anxiety and emotional reactions related to independent functioning which have been noticed to be affecting Anthony at home. Intervention: QMHP-C processed with Anthony about psychoeducational activity while assisting with directions. QMHP-C explained the importance of the activity. QMHP-C provided feedback to Anthony in regard to questions asked. QMHP-C explored Anthony’s responses and responded accordingly. QMHP-C talked with Anthony about the importance of adhering to rules and expectations in school, at home and in the community to ensure Anthony will commit to his therapy 4x a week. QMHP-C explored the Anthony’s capability to cope with his stressors and to get back on track with his objectives. Response: Anthony appeared to be learning from his mistakes and correcting them throughout the day. Anthony was acceptive of encouragement and decided to listen to his mom and do the right thing. Anthony realized his mom was trying to help him by pushing her to try on her own first before relying on others. Anthony focused on things he is good at and loves to do at home in order to stay motivated in the therapeutic setting. Anthony recognized his triggers and was able to identify common interests and similarities he and other ADHD recipients shared. Response: Anthony displayed partial participation during psycho-educational activity as evidenced by his frequent need to be refocused and minimal interaction during the processing conversational portion of the activity. Anthony has difficulty staying focused when distracted. Anthony understands that his actions have consequences and anything he does can reflect back to the way others treat him. Anthony demonstrated that he was able to process productive expressions of anger once he calmed down and learned to accept his faults without blaming others.
